Antibiotic treatment of bloodstream infections from different sources is one of the biggest challenges in infectology. A delay of up to 1-2 hours in the appropriate antibiotic treatment dramatically increases mortality, but at the same time, the microbiological diagnosis of these infections - especially the antibiotic sensitivity test of the pathogens that have been cultured - is time-consuming, so the appropriate choice of empirical treatment is highly emphasized. In addition to the general rules of the profession, this is significantly influenced by regional and even local resistance data, which significantly limit the automatic applicability of international practices and observations. A critical, comprehensive examination of the practice of empirical antibiotic selection in Hungary has not yet been carried out. The aim of the present project is to investigate the indications of empiric antibiotic treatment used in the case of bloodstream infections in the clinics of a university clinical center with a high degree of complexity and a wide spectrum of care, the factors influencing it, and the correctness of the decision in connection with the cases identified by blood cultures for bacteriological and mycological examination at the University of Pécs Clinical Center Institute of Microbiology. In the same way, in case of a positive culture result, we will examine its effect on the treatment, i.e. with these, the necessary change, escalation, de-escalation or non-escalation of the therapy, and finally we will follow the microbiological and clinical effectiveness of the applied treatment. Our aim is to compare domestic and international practice and, with the results, to develop proposals that can improve the effectiveness of the process, basis, and algorithms of empirical and targeted antibiotic selection.
